Alternatively, you can pick up a CHAS application form at any Restructured Hospitals, Polyclinics, Community Centres/Clubs (CCs) or Community Development Councils (CDCs).
If more than one family member from the same household is applying for CHAS, you will only need to send in one application form.
After completing the application form, please check that you and all your family members have done the following:
- Ticked the correct box indicating CHAS (i.e. first-time), disability or renewal applicant"?
- Attached clear copies of NRIC (front & back) of main applicant and all family members living in the same address (as reflected on their NRICs) and for family members below 15 years of age, clear copies of their birth certificates if NRIC is not available?
- All your family members signed the “Consent / Declaration” section?
- Completed the “Consent Exemption” section for your family member who cannot give consent?
(Please also include foreign identification documents for submission if your family members are not Singapore citizens.)

Your application will be processed within 15 working days from the date of receipt of the completed application form with all supporting documents. You will be notified of the application outcome by post.
